The annual Long Form Outcomes Framework (LFOF) survey will take place from 1 September to 16 December 2024.
Every year, the NDIS conducts this survey to measure how NDIS participants and their families and carers are progressing in different areas of their lives and compares the experiences of people with disability to the experiences of people without disability in Australia and other countries.
Participation in the survey is voluntary — you do not have to participate.
How do I participate?
The survey is conducted by Australian Healthcare Associates (AHA) on behalf of the NDIS. AHA will contact you directly:

If you have any doubt about the identity of the person contacting you, ask them for their name, phone number and the type of survey they are doing and then verify this by contacting the NDIA on 1800 800 110 before you do the survey.
